Speakers: Tracy Barbour, MD, and Rebecca Allen, MD, MPH, FCTMSS

This Grand Rounds provides a practical framework for evaluating TMS evidence—from randomized controlled trials through meta-analyses, clinical guidelines, and real-world practice. The speakers will examine study design, statistical versus clinical significance, generalizability, and how clinicians can determine which intervention works, for whom, under what conditions, and with what degree of certainty.

Key learning objectives include:

  • Analyze key elements of randomized controlled trial methodology, including sham conditions, coil verification, participant selection, and outcome measures.
  • Differentiate statistical significance from clinical significance by considering effect sizes, response and remission rates, durability, and applicability.
  • Integrate evidence from individual trials, meta-analyses, clinical guidelines, regulatory approvals, and real-world practice when evaluating TMS interventions.

Tracy Barbour, MD

Bio: Tracy Barbour, MD, is Medical Director of the Transcranial Magnetic Stimulation (TMS) Clinical Service and Associate Director of the Electroconvulsive Therapy (ECT) Service at Massachusetts General Hospital, and Instructor in Psychiatry at Harvard Medical School. She also serves as Program Director of the MGH Interventional Neuropsychiatry and Neuromodulation Fellowship and is co-owner of Boston Precision Neurotherapeutics, a private practice specializing in fMRI-guided accelerated TMS. Her clinical and research work focuses on interventional psychiatry and the treatment of complex and treatment-resistant psychiatric disorders, with particular expertise in TMS, ECT, and ketamine. Her research investigates biomarkers of neuromodulation response—including functional connectivity, heart rate variability, and alpha oscillations—toward advancing precision treatment selection in depression and related conditions.

Dr. Barbour is President of the Clinical TMS Society (CTMSS) and has previously served a Co-Chair of its Clinical Standards Committee. She currently contributes to both the Clinical Standards and Education Committees. She serves as Associate Editor of Transcranial Magnetic Stimulation, the official journal of the CTMSS. She is an experienced educator in TMS and interventional psychiatry, teaching clinicians through the CTMSS PULSES courses.

Bio: Rebecca M. Allen, MD, MPH, is Partner and Director of Research at Seattle Neuropsychiatric Treatment Center and Clinical Assistant Professor of Psychiatry and Behavioral Sciences at the University of Washington. A board-certified neuropsychiatrist, her clinical and research work focuses on interventional psychiatry and the treatment of complex and treatment-resistant psychiatric disorders, with particular expertise in TMS, ECT, ketamine and esketamine, VNS, and emerging psychedelic therapies. She serves as principal investigator for clinical trials evaluating novel pharmacologic and neuromodulation treatments for depression, PTSD, anxiety, and other psychiatric disorders. Dr. Allen is a Past President of the Clinical TMS Society (2023–24) and has previously served as its Treasurer, a member of its Board of Directors, and Co-Chair of its Insurance Committee. She currently serves as Co-Chair of the Clinical Standards Committee. She is an experienced educator in TMS and interventional psychiatry, teaching clinicians through the Clinical TMS Society’s PULSES courses and speaking on TMS, neuromodulation, and emerging treatments in psychiatry.
