I have recently purchased a TMS machine and I am in the process of getting everything in place to begin offering this service.  I own my own practice and I am the only provider.  Does anyone have consent forms they would be willing to share with me, as well as a seizure protocol?  Any other helpful forms or policies would be greatly appreciated.  Thank you so much for your support!
